iHealth Clinic is the quickest way to get the DL-180 form filled out (no need to bring form). A Medical Exam is required in PA to obtain a Learner’s Permit.

If under age of 18, your parent or guardian is required to give consent for the medical exam. Parent or guardian will also be required to fill out DL-180TD for application.

Get Payment Ready - forms of Payment (check, money order, credit card, debit card)
Step 4
Visit DMVV - bring: DL-180, Dl-180TD (if needed), Proof of Identity, 2 Proofs of Residency, Social Security Card
Step 5
DMV Exams: eye screening and Knowledge Test

What Medical Conditions May Interfere with a Driver's License:

Driving a car or riding a motorcycle takes constant attention and good reflexes. Having medical issues that can decrease your attention are concerning. It is important that if you have any issues, that you report them for the safety of yourself and the community.
